Sample Informed Assent and Consent Forms for Child Participation

As part of the Global Child Protection AoR's "Working Across Sectors" workshop package, specifically Module 4 on Child Participation, these sample informed assent and consent forms can be adapted within your context when involving children throughout the programme cycle.  Other sectors may want to contact the Child Protection Coordination Group in your location to support adaptation of the forms along with the overall assent/consent process, risk assessment process, and any other aspect of safe and ethical child participation.   

The consent form would be used with parents or other responsible caregivers for participants under the age of consent.
